Model Search Application:
Please print out and complete the following form. All applicants must include with their application, any photos/slides/video/CD or DVD (not returnable). There is a $25 application fee to process and review your application if you do not have a sponsor code.
RULES: High School or College Students, full or part-time, currently enrolled, recently graduated, or planning to attend, submit application and materials by February 15. Applications received after this date will automatically be placed into the following years' search. Applicants under the age of 18, if selected, will require a parental release. We suggest that you send in your application as early as possible for maximum consideration.
SPECIAL NOTE: IF you have a "Sponsor Code" , the application fee is paid/covered by that sponsor... please list it below and you may also copy/paste this application form, and email to us with one swimsuit photo or similar to: classmateusa@bellsouth.net
ALL paid applications (with no sponsor code) must be sent in by regular mail with the $25 application fee.
